Need experts on investment goal setting

I’m looking for financial advisors and academics with tips on how investors should set their investment goals, especially if they work with a pro. First, what elements should be in an investment plan — a targeted return, inflation assumption, diversification, limits on fees, etc.? What kind of gauge should one use to assess acceptable risk? What if there are multiple goals, like college, a second home and retirement? If one works with a pro, should the investment strategy and targets be in writing? Should there be a provision for what happens if the plan falls short — a rebate on fees, for instance Requirements: I prefer responses by email containing usable content and quotes, and because I usually get plenty I rarely follow-up on responses that merely offer a source for interview.
